Information handled through this site
The public site does not provide a patient-record upload. Contact, checklist, and newsletter forms may collect business contact information, organization, request type, consent choices, and any business-level message you choose to provide. Do not submit patient information or claim identifiers.
The review-preparation tool evaluates your selections in your browser. The page does not send those answers to PRP. Do not enter patient information into any public website, scheduling field, or URL query string.
Contact requests, checklists, and IDR updates
PRP uses business contact information to respond to the request you submit, provide the requested resource, operate the contact workflow, and maintain a record of the permissions selected with the form.
Consent to receive occasional IDR updates is optional and separate from permission to answer a contact request. Marketing messages should provide a way to unsubscribe. PRP may retain contact information while a request is active, while you remain subscribed, or as reasonably needed for legitimate business and legal purposes.
Scheduling
If you continue to Calendly, the information you provide is handled by Calendly under its own terms and privacy practices. PRP asks for business-level scheduling information only. Do not include patient names, member IDs, claim numbers, dates of service, or clinical details.
Analytics and cookies
PRP does not place advertising cookies through this site. Cloudflare may process ordinary request and performance information to deliver, secure, and measure the site. PRP also records limited first-party funnel events, such as opening a scheduling handoff or completing a non-PHI contact form. Those events do not contain form messages, email addresses, or claim information.
Cloudflare Turnstile processes browser and security signals to distinguish legitimate form submissions from automated abuse. PRP receives the verification result rather than a visitor profile.
How information is used and shared
Public-site technical information may be used to operate, secure, troubleshoot, measure, and improve the site. Business contact information may be used to respond and, where optional marketing consent was selected, send PRP updates. PRP does not sell personal information collected through this public site. Service providers may process limited information on PRP’s behalf for hosting, data storage, scheduling, or communications.
Client records and PHI
Client records are governed by the applicable services agreement, business associate agreement, and PRP’s separate secure application—not by a public website submission. See the Security & PHI page.
Questions and updates
Use the contact page for privacy questions, correction or deletion requests, or to withdraw marketing consent. PRP may update this notice as the site, vendors, or operating model changes. The effective date above will be revised when material changes are published.
